3. If you prefer to use the on line worksheet and pay by electronic check, just click on the on-line option, fill out the appropriate form fields and submit. (If your information requires more text fields than the form allows, please use our supplemental form for your category which can be located on the Services page.
   
   
4. If you prefer to fill out the worksheet by hand and then mail the worksheet, click on the download PDF (printable file) option. After printing the worksheet you may then mail the worksheet with a personal check or money order, or charge by credit card.
   
   
5. Once the worksheet is complete and payment is received Nationwide Court Documents will process your order.
